SOLAR is a DHS funded Recuperative Care Program serving individuals experiencing homelessness that also have acute medical issues. The RN Care Coordinator will take a leadership role in leading the team of LVNs in the Recuperative Care program and will ensure the quality and adequacy of nursing services provided to members. Continue treatment plans from the referring entity. Provide medical guidance for LVNs in coordinating PCP appointments and/or other medical follow ups upon admission. Provide care, and treatment plans to individual members (38 Clients). Monitor, reevaluate and manage medical conditions on a routine basis as needed. Provide medication management to individuals including but not limited to medication monitoring, patient teaching, and supportive services to increase compliance with medication regimens. Provide wound care to members as needed and/ train medical team. Document and chart all services provided in chart and electronic database with accuracy and in real time. Promote patients' independence by establishing patient care goals, teaching patients to understand their medical condition, medications, and support self-care skills. Work in collaboration with the rest of the SOLAR team- case managers, and mental health provider, as appropriate, of any medical and/or medication concerns. Maintain medication logs and medication storage in compliance with safety standards and industry standards of care. Provide crisis intervention assistance when needed. Be able to notify the appropriate community agency in an emergency (i.e., police, paramedics, fire department). Maintain open communication with direct supervisor, individual and group supervisor(s). Participate in staff meetings, nursing meetings, and supervision. Ensure that medical services are reflective of the agency's treatment model, which emphasizes member choice, harm reduction, trauma-informed care, and psycho-social rehabilitation. Conduct Health/Wellness and medication management groups.     Job Description Skin by Lovely is seeking talented, suitably licensed medical providers with a passion for facial aesthetics, skin care, and paramount safety standards. Responsibilities include providing diagnostic and therapeutic care for facial aesthetic patients including taking medical histories, performing examinations, administering procedures, and developing a treatment plan for top-notch patient care as directed by the Medical Director. By keeping a focused treatment menu and providing ongoing and regular training, Skin by Lovely is consistently ranked a top practice in the U.S. by Allergan, and Galderma. Through our sister company, InjectionU, and the support of other educational providers, we are also able to act as an educational center by providing comprehensive, hands-on training, for health care providers new to medical aesthetics, including newly hired Skin by Lovely Injection Specialists. Education & Experience: Experience in a medical office working as a facial aesthetic provider Registered Nurse or Nurse Practitioner Current (California) license in good standing Essential Skills and Abilities: Possesses wide-ranging knowledge of facial aesthetics and skin care treatments. Good problem solving and analytic skills. Excellent communication skills. Ability to work as a team member. Well-organized with attention to detail. Ability to manage multiple projects simultaneously. Treats patients and co-workers with respect. Empathetic with patient’s needs and concerns. Duties and Responsibilities: Conduct consultations with patients to determine best treatments for their desired outcome and create a plan of care within the practice guidelines. Carry out treatment plan with confidence, expertise, and appropriate usage and quantity of product while providing a safe and clean treatment regimen per practice protocols. Handle patient questions and concerns professionally and courteously. Provide appropriate follow-ups and timely responses to all treated patients. Maintain an energetic positive attitude and contribute toward a quality and peaceful work environment.
