We are seeking a dedicated and compassionate RN Care Coordinator to join our team. SOLAR is a DHS funded Recuperative Care Program serving individuals experiencing homelessness that also have acute medical issues. The RN Care Coordinator will take a leadership role in leading the team of LVNs in the Recuperative Care program and will ensure the quality and adequacy of nursing services provided to members. The RN provides on-site medical management and health education to participants, and liaisons to community and hospital partners within the RNs scope of practice
Duration: 13 weeks contract
(possibility of temp-to-hire or extension depending on needs)
Schedule: 40 hours/week; Monday- Friday 8:00 am - 5:00pm
Pay: $ 44.23 - $50.48/ hour

Active RN license in the state of CA

Management/Charge Nurse experience, familiar with EXYM is super helpful. *There are up to 38 clients in the location at any time, there are LVNs to support.*

Experience working with individuals with mental health and substance use issues.
Current valid California Driver's License
Preferred Qualifications:
Experience working with those living with mental illness and/or substance addictions.

Provide wound care to members as needed and/ train medical team.
Document and chart all services provided in chart and electronic database with accuracy and in real time.
Promote patients' independence by establishing patient care goals, teaching patients to understand their medical condition, medications, and support self-care skills.
Work in collaboration with the rest of the SOLAR team- case managers, and mental health provider, as appropriate, of any medical and/or medication concerns.
Maintain medication logs and medication storage in compliance with safety standards and industry standards of care.
Provide crisis intervention assistance when needed.
Be able to notify the appropriate community agency in an emergency (i.e., police, paramedics, fire department).
Maintain open communication with direct supervisor, individual and group supervisor(s).
Participate in staff meetings, nursing meetings, and supervision.
Ensure that medical services are reflective of the agency's treatment model, which emphasizes member choice, harm reduction, trauma-informed care, and psycho-social rehabilitation.
Conduct Health/Wellness and medication management groups.
Maintain confidentiality of member files per HIPAA and all applicable guidelines

Duties and Responsibilities:
Conduct consultations with patients to determine best treatments for their desired outcome and create a plan of care within the practice guidelines.
Carry out treatment plan with confidence, expertise, and appropriate usage and quantity of product while providing a safe and clean treatment regimen per practice protocols.
Handle patient questions and concerns professionally and courteously.
Provide appropriate follow-ups and timely responses to all treated patients.
Maintain an energetic positive attitude and contribute toward a quality and peaceful work environment.

Taking part in training and other learning opportunities to expand knowledge of company and industry.
Accurately record client treatments in EMR the day of treatment
Actively promote the practice, treatments, and services, as well as programs, promotions, and discounts available
