Responsibilities

Requirements
Minimal of three (3) years experience in chronic care or family/primary medicine as an APP
Must hold an active unrestrictive license as an Nurse Practitioner or Physician Assistant
Ability to provide compassionate care and establish patient rapport
Strong understanding of examination methodologies to include lab work, images, and patient medical records and diagnostics and their interpretation.
Broad knowledge of common medication, side effects and contraindications
Respect for patient’s confidentiality and desires for their own healthcare
Oncology experience as a Nurse, PA or NP is preferred
Must be authorized to work in the United States
Pay Range is $45- $70/hr
The shift for this position is four 10 hour days and includes rotating weekend requirements.

The Opioid Epidemic is a public health crisis with a highly effective but underutilized clinical intervention - millions of Americans are physically dependent on Opioids but only 10% of those likely to have OUD actually access treatment. Bicycle Health addresses this gap by maximizing accessibility, affordability, and overall quality of care by enabling highly qualified clinicians to reach patients broadly and efficiently through our online platform.
We're looking for Nurse Practitioners and Physician Assistants to join us full-time in a 40-hour-per-week, W-2 role with a schedule designed to offer both structure and flexibility.
We are not currently hiring 1099 or part time providers.
The Provider position at Bicycle Health is a remote full-time addiction medicine clinical position with responsibility for the continuity care of a panel of adult patients under care primarily for opioid use disorder. The Nurse Practitioner or Physician Assistant will assess new patients presenting for telemedicine-based care of opioid use disorder, determine appropriateness of enrollment in our care setting, and manage their continuity OUD treatment from initial induction through the duration of their care. The Nurse Practitioner also addresses co-occurring mental health conditions directly when appropriate, and also via referral and care-coordination with outside providers. The NP/PA participates actively in regular clinical team meetings and quality improvement activities, collaborates with our program physicians and therapists, and reports to the regional medical director.

What you'll Do:
Full-time (40 hours per week) opportunity.
Provide assessment and treatment for a continuity of panel of up to 275 adults with Opioid Use Disorder from initial treatment-start throughout their ongoing care
Provide diagnosis of Opioid Use Disorder and co-occurring mental health conditions
Safely and effectively start treatment with buprenorphine-naloxone for OUD, manage the early induction and stabilization phase of care with close follow-up
Utilize motivational interviewing and other engagement and rapport-building techniques to achieve optimal patient outcomes

Take an active role in clinical team meetings and practice improvement, engage in case discussion with colleagues

GENERAL STATEMENT OF JOB
The Care Manager Extender, hereafter referred to as Extenders, will support care managers in delivering Tailored Care Management and fulfilling the member contact requirements. Extenders will help Vaya Care Management best meet the needs of members, build efficient care teams by creating additional workforce capacity, and allow care managers and supervisors to focus on key tasks for assigned members as well as permit them additional time for members with intensive or complex needs. Certain components of Tailored Care Management will always be led by a care manager. However, some functions may be performed by an Extender as part of Tailored Care Management.
Extender will report to Care Manager and remain within the scope delineated in this job description and only performing functions within their training, scope, and abilities. Extenders will participate with the Care Manager and care team in supporting the member and integrate care team's goal in a holistic approach and document interactions and activities within the identified administrative healthcare record according to departmental standards. Supervising care managers, care managers, and extenders will be in regular communication and coordinating efforts to mitigate duplicative or inappropriate outreach to assigned members.

ESSENTIAL JOB FUNCTIONS
Member Outreach, Support & Education:
Extenders may support care managers in delivering Tailored Care Management by performing activities that fall within the below categories:
Performing general outreach, engagement, and follow-up with members.
Coordinating services/appointments (e.g., appointment/wellness reminders, arranging transportation).
Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing; such as providing education on members' chronic conditions; teaching self-management skills and sharing self-help recovery resources; Providing education on common environmental risk factors including but not limited to the health effects of exposure to second and third hand tobacco smoke and e-cigarette aerosols and liquids and their effects on family and children; and Promoting wellness and prevention programs.
Sharing information with the care manager and other members of the care team on the member's circumstances.
Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services.
Participating in case conferences.
Support the care manager in assessing and addressing unmet health-related resource needs.

QUALIFICATIONS & EDUCATION REQUIREMENTS
High school diploma or GED is required for this position. Must be at least 18 years of age and trained in Tailored Care Management. Must meet one of the following requirements:
Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system; or
Be a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist; or
A parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (parent/guardian cannot serve as an extender for their family member); or
Has two years of paid experience performing the types of functions described in the “Duties/Responsibilities section of this role description in addition to one year of paid experience working directly with the Tailored Care Management eligible population.
Licensure/Certification Requirements:
None required. Certified PSS (Peer Support Specialist) and/ or WRAP (Wellness Recovery Action Plan) Facilitators preferred.
MENTAL/PHYSICAL REQUIREMENTS
Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading.
Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists and fingers.
Sedentary work with lifting requirements up to 10 pounds, sitting for extended periods of time.

Job Summary:
Provide primary care to children, adolescents, and adults in a private practice outpatient setting, to include annual visits, weight management support, psychiatric evaluation/assessment, medication management, supportive counseling, and treatment coordination with other providers in the practice. A minimum of 35 patient hours per week (average of 7 patients per day) salary based on number of weekly billable patient hours the candidate completes.
The provider will have all administrative supports (clinical assistant, front desk staff, credentialing support, billing, access to printer/fax/voicemail etc.) provided. A furnished, private office will be available, as well as clinical support from our team of psychotherapists. We utilize TheraNest for electronic medical records, patient billing, e-prescribing, and patient portal. Additionally, we utilize a HIPAA-compliant version of Teams and SharePoint for 24/7 access to colleagues, including multiple channels that are both professional and non-work related supports.
Employee will be reimbursed, in full, biweekly based on an annual salary rate as a W2 employee. Benefits include health/dental/vision insurance, 401k with employer match, paid time off, annual training support, and malpractice insurance.
Qualifications & Skills:
The successful candidate will be licensed in Virginia to practice as a Physicians Assistant in the Commonwealth of Virginia; hold current certification by the American Nurses Credentialing Center as a Psychiatric and Mental Health Nurse Practitioner; hold a prescriptive authority for Nurse Practitioner in Virginia; have working knowledge of assessment, diagnosis, treatment/planning; have demonstrated ability to communicate effectively with patients, families, staff and other health care professionals; have the ability to document findings accurately, thoroughly, and promptly; possess the ability to recognize legal limits of practice; demonstrate experience obtaining and interpreting clinical data and draw satisfactory conclusions; be capable of assessing emergencies and taking appropriate actions.
