Position Overview:

Burnsville Family Physicians is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.
Essential Functions:
Escorts the patient to the Clinical Support area. Take height and weight. Verify visit reason.
Checks immunization record and update as needed.
Inquires about medications, allergies, and smoking status at each visit.
Take BP, temperature, pulse, respiration, head and chest measurement pertinent to exam.
Gowns patient appropriately for exam.
Prepares room as needed for specified visit type.
Schedules emergent tests as ordered by the provider.
Hands out patient education pamphlets or print patient education from EPIC as requested
Processes referrals by giving information to the patient for scheduling.
Assists with laceration and set up surgical tray.
Assists with excision of moles or lesions. Give specimen to lab for send out. After the pathology report is received, notify the patient of the results per provider request/instruction.
Assists patient with completing informed consent forms prior to the provider performing the procedure(s) and send to Medical Records.
Orders injections/immunizations as appropriate.
Administers injection, SQ, IM, and ID.
Retrieves calls regarding patient health problems, documents and routes calls.
Calls in new prescriptions and refills to the pharmacy after provider has reviewed and approved.
Calls patient with normal lab results, or normal results that have been reviewed by the physician with specific instructions by provider.
Checks provider's out box and review all papers in the box to ensure they are initialed by the provider prior to going to medical records for scanning.
Fully participate and contribute to the patient centered health care home.

For over 40 years, the Minneapolis Health department has continued to pride itself in operating in a multi-disciplinary clinic in 9 Minneapolis High schools to improve and promote the health and well-being of adolescents. The Nurse Practitioner/Physician Assistant (NP/PA) provides clinical services and overall
coordination of clinic operations at individual clinic sites for the School Based Clinic (SBC)
Program. The position is responsible for providing primary medical care to adolescents, within
the scope of the MN Nurse Practice Act and MN Board of Nursing regulations or medical
practice board and physician delegation agreement. The NP/PA provider consults with the
medical director and refers clients as needed for further assessment or care.
This position collaborates with a multi-disciplinary team of other professionals (mental health,
health educator, dietician, MA, MD) who all contribute to restoring, maintaining and promoting
the health and wellbeing of our adolescent population.
There is one full-time exempt vacancy in the Minneapolis Health Department. This position
reports to the Manager of School Health Services for the School Based Clinic program, with
medical supervision provided by the program's Medical Director.
SBC is a current National Health Service Corp (NHSC) site which qualified candidates
are eligible for loan repayment for the SBC employment.Provision of Clinical Services
* The provider is responsible for assessing physical status of SBC clients by taking and
evaluating health histories, performing complete and partial physical exams, assessing
acute illnesses or injuries; assessing current or potential chronic diseases; ordering
laboratory testing, evaluation results and recommending treatment; and reviewing and
administering immunizations.
* The provider is responsible for the appropriate diagnosis, treatment, counseling, and
follow-up of clients based on their assessment.
* Document the health history and medical care provided in electronic health record
(EHR). Treat illness and other abnormal clinical findings according to best practices and
protocols established in collaboration with the medical director.
* Write prescriptions and dispensing over the counter and stock medications from the
clinic according to clinic guidelines.

* Review client electronic health record documentation and billing encounter forms for

The new faculty member will be asked to expand our epilepsy surgery program in collaboration with other division faculty, pediatric neurosurgery faculty, and adult epilepsy faculty. A full array of outpatient and inpatient neurophysiology resources is available as well as a comprehensive team of pediatric neuropsychologists, neuroradiologists, psychiatrists, geneticists, and ketogenic dieticians. The epilepsy surgery program capabilities include Stereo-EEG, ROSA, VNS, LITT, RNS, and DBS. There will be numerous opportunities to engage in clinical research activities. Other areas of subspecialty expertise in the division include neuromuscular disorders, headache, neuro-immunology, neurogenetics, and neonatal neurology.
Our division will launch a new pediatric neurology residency program in 2025, reflecting our faculty's strong commitment to education. Our team is actively involved in teaching medical students, adult neurology residents, and pediatric residents with ample opportunities to mentor trainees at all levels. As a key part of the Department of Neurology, our division benefits from and contributes to the department's vibrant research, education, and clinical programs. We also collaborate actively with the Department of Pediatrics in all these domains, and have strong relationships with other departments.

Pay and Benefits:
This role is dually employed by University of Minnesota Physicians and the University of Minnesota. Total salary is market competitive and based on AAMC salary benchmarks. Clinical salary, aligned with clinical effort, is provided through M Physicians. The base salary range for this position is $210,017 - $283,100 commensurate with experience, qualifications, and cFTE. The salary from the University is dependent upon the academic effort of the person hired.

For over 40 years, the Minneapolis Health department has continued to pride itself in operating in a multi-disciplinary clinic in 9 Minneapolis High schools to improve and promote the health and well-being of adolescents. The Nurse Practitioner/Physician Assistant (NP/PA) provides clinical services and overall
coordination of clinic operations at individual clinic sites for the School Based Clinic (SBC)
Program. The position is responsible for providing primary medical care to adolescents, within
the scope of the MN Nurse Practice Act and MN Board of Nursing regulations or medical
practice board and physician delegation agreement. The NP/PA provider consults with the
medical director and refers clients as needed for further assessment or care.
This position collaborates with a multi-disciplinary team of other professionals (mental health,
health educator, dietician, MA, MD) who all contribute to restoring, maintaining and promoting
the health and wellbeing of our adolescent population.
There is one full-time exempt vacancy in the Minneapolis Health Department. This position
reports to the Manager of School Health Services for the School Based Clinic program, with
medical supervision provided by the program's Medical Director.
SBC is a current National Health Service Corp (NHSC) site which qualified candidates

Job Duties and Responsibilities
Provision of Clinical Services
• The provider is responsible for assessing physical status of SBC clients by taking and
evaluating health histories, performing complete and partial physical exams, assessing

laboratory testing, evaluation results and recommending treatment; and reviewing and
administering immunizations.
• The provider is responsible for the appropriate diagnosis, treatment, counseling, and
