This hematology - oncology position will include areas of responsibility for all phases of caring hematology and oncology patients. Scope of JobProvides Pharmaceutical care to adult kidney and liver transplant patients within the first year of transplant and those being followed by the Royal Oak transplant team. Rounds and collaborates with the transplant team on 10-20 patients daily including order verification, management of Pharmacist concults and medication review. Provides essential medication eduation to transplant patients, prior to discharge, to assist with transitions of care and decrease readmissions. Completes 5-20 pre-transplant donor and recipient evaluations weekly.Provides leadership for all Pharmacy clinical activities within assigned area. Embraces the goals/objectives encompassing Pharmaceutical activities. Promotes team efforts with pharmacy and non-pharmacy personnel to manage and achieve departmental/corporate objectives. Provides support for staff development so that staff meets/exceeds minimum competency standards and encourages/supports staff to continue professional development. Demonstrates good judgment and decisiveness throughout daily activities. Maintains knowledge of current pharmaceutical practice and demonstrates a personal commitment to professional growth. GENERAL SUMMARY: Eligible for Part Time Benefits The pharmacist assumes responsibility for the day-to-day comprehensive, patient-oriented pharmacy services provided on his/her shift in each area of the department. The pharmacist works under the general direction of the area's administrative or clinical coordinator and provides direction and supervision for supportive staff assigned to his/her shift. Should have the skills necessary for providing informal and formal instruction of students and other health care providers on drug therapy and inpatient operational activities. * Must demonstrate the ability to work with others in developing improved services for patients. * CERTIFICATIONS/LICENSURES REQUIRED: * Must be licensed or eligible for licensure in the State of Michigan.
