Basic word processing skills and able to operate hospital programs used in the Pharmacy

Prepares large volume parenterals, IV piggybacks, total parenteral nutrition, chemotherapeutic and antineoplastic agents, IV push medications, irrigations, patient controlled analgesics, ophthalmic preparations, and enteral feedings.
* Assists with overseeing the work of Unit dose and IV Additive Technicians the in performance of their regular duties. Ensures that all drugs and solutions prepared by technicians are accurate and prepared under proper conditions. Checks and verifies accuracy of medications in carts; releases carts for delivery to patient care areas.
* Prepares and checks stock drugs for nursing units and other departments. Monitors pharmacy and nursing unit medications on a regular basis in order to see that all drugs are current and in adequate supply.
* May provide pharmacokinetic evaluation and dosing of patients on specific medications and monitors patient progress. Evaluates and acts on laboratory data and patient response.
* Maintains an up to date knowledge of developments and advances in the pharmacy field; fulfills continuing education requirements in order to maintain registration.
* Maintains work area and equipment in a clean and orderly condition.

POSITION SUMMARY:

DUTIES AND RESPONSIBILITIES:
Accepts, validates, interprets and processes prescription orders.
Initiates and completes calls with physicians regarding appropriate drug therapy.
Conducts Drug Utilization Review (DUR) and provides necessary clinical interventions.
Provides physician education and information/counseling; reconciles drug therapy management issues.
Responsible for Formulary Management - including Therapeutic Interchange or Substitution, and Coverage Review Programs (e.g. Prior Authorization).
Ensures quality controls for pharmacy practice by following Standard Operating Procedures and all legal requirements set by the DEA and state pharmacy board.
QUALIFICATIONS:
Must be licensed to practice in the State of Michigan.
Consideration will be made for a recent college graduate who has performed Internship or Residency.
Experience in a retail/community practice setting is preferred.
Must be comfortable in discussing drug therapies with providers.
Must be willing to maintain and acquire professional skills through continuing educational resources.

PHARMACIST IN CHARGE:
The following job description identifies the qualifications and standards for the Pharmacist in Charge (PIC). The PIC will meet the following requirements and competencies listed below. The PIC reports directly to the Director of Outpatient Pharmacy.
Job Summary: The Pharmacist in Charge (PIC) is responsible for planning, organizing, and directing the overall operation of the Pharmacy Department. The PIC shall manage various operations within the pharmacy including, but not limited to:
❖ Supervises personnel and participates in all aspects of pharmacy workflow including data entry, prescription fulfillment, final verification, and dispensing.
❖ Ensures compliance with patient care quality standards as it relates to the care provided to all age groups of patients.

❖ Participates in the department’s planning, revenue analysis, budgeting, education, and human resource activities.

The PIC will have strong communication skills and be able to perform detailed work. Work is accomplished in accordance with the policies and regulations of local, state, and federal authorities as well as accepted standards of practice.
Duties and Responsibilities:
Distributive Activities:
❖ Prepares, labels, and dispenses medications for all age groups according to accepted standards of practice.
❖ Performs computer order-entry functions in an accurate and timely manner.
❖ Assists in maintaining an acceptable inventory of medications and supplies under proper
conditions of storage and security.
❖ Supervises and verifies appropriate and accurate drug distribution tasks performed by support and technical staff.
❖ Effectively uses all necessary forms of informatics.
❖ Performs other duties as needed or assigned.
Direct Patient Care Activities:
❖ Performs all aspects of pharmacy operations in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.
❖ Interprets medication orders, monitors drug regimens, and performs interventions when order clarification is necessary or when therapeutic problems are identified.
