Job Title: Intensive Care Manager I (Murfreesboro, TN)
Schedule: Mon-Fri, 8:00a-4:30p
Salary: $43,500 (base pay) with additional incentive for language skills and behavioral health experience. Mileage reimbursement available.
JOB SUMMARY: Provide continuous treatment team care management services to children and youth with serious emotional disturbance or severe mental illness, their families, and other team members' consumers. Mentor new care managers on basic skills.
RESPONSIBILITIES:
Continual assessment of consumers' needs, and development of individualized service plan and crisis plan with measurable goals to ensure needs are met.
Assist with referral and linkage to community resources and advocate for consumers whenever necessary.
Ensure consumers are seen upon discharge from the hospital and Diversion Services within Agency standards.
Ensure consumer's daily psychiatric, medical, and environmental needs are planned for and coordinated.
Ensure efficient transition of consumers when needed within the Agency and the community.
Meet productivity standards as indicated in the Policies and Procedures.
Meet regularly with Care Management Supervisor (individual and group).
Attend morning and weekly team meetings as scheduled. Follow-up with the plan(s) developed in each meeting. Attend quarterly All Care Management meetings/trainings.
Facilitate education of consumer and/or involved collaterals to psychiatric diagnosis and appropriate treatment options.
Respond to urgent/crisis consumer needs as appropriate to position with assistance of supervisor, crisis services, and clinic staff by developing plans and interventions
Develop and maintain relationships within family unit and supports.
Ensure timely and appropriate documentation as required by the Agency.
Develop and maintain community relations through prompt follow through with contacts.
Continual involvement in the family system, school system and other systems in which the family may be involved (juvenile justice, other supportive agencies, etc.).

REQUIREMENTS:
Bachelor's Degree in a health-related field of counseling, psychology, social work or other behavioral health field.
Candidates with a bachelor's degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.

WHAT YOU’LL DO
Comprehensive Patient Care:
Conduct thorough health assessments, including obstetric, gynecologic, contraceptive, medical, surgical, and psychosocial histories.
Perform physical exams with a focus on reproductive health, including breast and pelvic exams, cancer screenings, and STI diagnosis.
Provide counseling and education on contraception, fertility, sexual health, and overall wellness.
Conduct and interpret diagnostic studies, lab tests, and ultrasounds per medical guidelines.
Deliver patient-centered care and provide follow-up as needed, seeing an established patient visit goal/productivity expectations, seeing approximately (30-35) patients per day with a combination of scheduled appointments (10-15 minute increments) and walk-ins.
Ensure patient care aligns with medical standards and guidelines (MSGs), and protocols as outlined by the affiliate to meet accreditation standards.
Respond to after-hours patient calls as part of the on-call team.
Clinicians will see anywhere from 25-35 patients per day with a mix of scheduled appointments and walk-ins.

WHAT YOU BRING
Education & Certification:
Master’s Degree in Nursing (MSN) or equivalent education in Physician Assistant or Nurse Midwifery programs.
Minimum of three years experience as a Nurse Practitioner, Physician assistant or Nurse Midwife preferred.
Active and unrestricted Tennessee Nursing License.
National Certification as a Nurse Practitioner, Physician Assistant, or Certified Nurse Midwife.

Position Summary:
Provides care coordination services to families who have been diverted from states custody through the Child Protective Services system. Responsible for creating and/or monitoring the non-custodial permanency plan and ensuring that adequate services and assistance are provided to youth and families to complete the action steps on the plan. Ensuring that placement is stable and meeting the needs of the youth and coordinating needed placement with the main purpose of preventing a custodial placement with the state. Primary liaison between team members including; provider agencies, courts, schools, attorneys, guardian ad litem, CASA, provider services, biological families or caregivers and any other involved parties.
Essential Duties and Responsibilities:
* Carry caseload of approximately 20 cases.
* Serves Rutherford, Wilson, and Davidson Counties; regional travel to other surrounding counties will be required.
* Work flexible hours including weekends, holidays and after hours, including some on-call rotation.
* Work in a team setting.
* Learn and have competency with Department of Children's Services policies that guide care and services for youth and families.

* Conduct frequent home and site visits. Assess safety needs of homes or placement site. A minimum of twice per month in person is required. Some situations will require more frequent in person visits. Travel will be required for home/site visits throughout the state of Tennessee.
* Prepare for, organize, notify other team members and lead Team Meetings.
* Prepare for, organize, notify other team members and participate in court hearings. Ensure all court documents are turned into the judge per the counties court requirements.

Establishes and maintains working relationships with community resources and provides appropriate linkages for service members and their families. Partners with POC to provide MFLC services in a manner that addresses the needs of the installation. Develops an effective and professional working relationship with the installation POC.
* Provides MFLC response to critical incidents, surge and on demand events, and special requests as directed by the POC and approved by the Office of Secretary of Defense (OSD) program manager. Counselor will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director.
* Manages duty to warn and restricted reporting situations according to Department of Defense (DoD) protocol and staffs the cases with Regional Supervisor/Regional Director.
* Communicates with Regional Supervisors and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations to which the counselor is assigned.
* Participates in regular in-services/training, Quality Improvement committees or other contract activities as assigned/appropriate.
* Participates in initiatives, studies, and pilot programs as directed by the customer and/or Magellan. This includes participation in pilot program certification and training processes, completion of activity documentation, integration of pilot activities in non-medical counseling work, and the utilization of applicable technology to complete required activities.
